What the ride request form collects

When you request a ride on this website, we collect the information you type in:

• Your name, phone number, and (optionally) email address
• Pickup and drop-off addresses
• Trip date and pickup or appointment time
• Optional trip details: vehicle type, round trip and return time, number of passengers, and any notes

If you contact us by phone, text, or email, we receive the information you choose to share.

How we use it

We use this information only to schedule and provide your ride and to contact you about it, for example to confirm your pickup by phone or text.

We don't sell your information

We do not sell, rent, or trade your personal information. The form is delivered to us through our website host's form service so that we can receive your request.

Please don't send medical details

Our online form and email are for trip details only. Please don't include medical information such as diagnoses or conditions. If we need to know about mobility needs, like a wheelchair, we'll talk with you by phone.

Wisconsin Medicaid rides

Medicaid rides are booked through MTM at 866-907-1493. Information you give MTM is handled under MTM's own policies.
